- 1st Member — $349
- 2nd Member — $299
- Additional Member — $249

- Routine x-rays

- 2 cleanings or periodontal maintenances
- 20% discount on most services*

Plan Guidelines
- Patient portion is due the day of service
- Cannot be used in conjunction with other dental plans or financial programs such as CareCredit or Cherry
- No refunds of premiums will be issued if member decides not to utilize the membership plan
