Annual Membership Costs
1st member - $250
Each additional member - $175
Renewal fee - $150
Includes:
- Up to 2 free dental exams
- Routine X-rays
- 1 Emergency exam
- 50% off basic cleaning and
periodontal maintenance - 20% discount on most services*
*Exclusions apply
Plan Guidelines
- The patient's portion is due on the day of service.
- This plan cannot be used in conjunction with other dental plans or financial programs such as CareCredit and Sunbit.
- No refunds of premiums will be issued if the member decides not to utilize the membership plan.
